Assessing the Fragire grid on the basis of Share survey data

In 2014 Cnav’s Social Action department decided to develop a new frailty detection tool to improve the targeting of individuals at risk of losing their autonomy and provide them with prevention programmes better adapted to their needs. The new tool, called the Fragire grid, was devised by the inter-regional gerontology centre of Bourgogne Franche-Comté (PGI) on the basis of a clinical cohort. In this study, we seek to assess the extent to which the Fragire score is 1) reproducible with the general population, 2) able to identify frail seniors by using the Fried phenotype as a reference indicator, and 3) capable of predicting the initiation of autonomy loss (the emergence of restrictions in everyday activity and the development of personal care). We draw on the data of the Share survey panel ( Survey on Health, Ageing and Retirement in Europe, with individuals aged 50 and over) representative of the ordinary household population in metropolitan France between 2004 and 2020. These data are used to rebuild the algorithm of the Fragire grid according to the methodology developed by the PGI and to measure the correspondence of the Fragire score with the variables related to the loss of autonomy in two- to four-year intervals. Our results suggest that, while not perfect, this score does indeed serve to detect fragile individuals. It can also be used to anticipate the recourse to care and future incapacities. The findings underline the importance of this new indicator of frailty with respect to the development of effective prevention policies able to target individuals at risk of losing their autonomy.
