Die Arbeit der Entstellung setzt voraus, dass die Verdrängung und ebenso die Differenzierung zwischen dem Bewusstsein und dem Unbewussten in der psychischen Struktur vorhanden sind. Bei der psychotischen Funktionsweise besteht jedoch diese Differenzierung von Bewusstsein und Unbewusstem nicht, was zu projektiven Transformationen führt.
