RésuméLa Maison de Santé Léonard de Vinci est née d’une volonté commune entre la médecine de ville, les élus et l’hôpital. Projet de soins pluriprofessionnel agréé par les tutelles, elle regroupe 25 professionnels et une association de patients. Des partenariats ont été signés avec la caisse primaire pour « Mission : retrouve ton cap », avec la Mutualité Sociale Agricole pour les suivis complexes. L’irruption de la COVID-19 a accéléré la mutation de la maison de santé en laboratoire d’applications. Le centre SOS COVID-19, de dépistage et de soins premiers, a effectué 451 consultations dont 17 % de téléconsultations lors de la première vague, 400 prélèvements lors la troisième vague. Le centre de vaccination, regroupant 80 professionnels libéraux, salariés, internes et retraités, couplé à la pharmacie de l’hôpital, a effectué 10 000 injections de vaccins ARN-messagers en trois mois. Transcendant les statuts par sa mission de santé publique, la vaccination est apparue ainsi comme un levier de collaboration ville-hôpital promettant de nouveaux partenariats et un rôle pivot pour la maison de santé dans son territoire sanitaire.

The Leonard de Vinci Health Center was born out of a common desire between city medicine, elected officials and the hospital. Multi-professional care project approved by the supervisory authorities, it brings together 25 professionals and an association of patients. Partnerships have been signed with the primary fund for “Mission regain your course”, the “mutualité sociale agricole” for complex follow-ups. The outbreak of COVID-19 has accelerated the transformation of the nursing home into an application laboratory. The SOS COVID-19 center, for screening and first aid, carried out 451 consultations, of which 17% were teleconsultations during the 1st wave, 400 samples during the 3rd wave. The vaccination center, bringing together 80 liberal professionals, employees, interns, and retirees, coupled with the hospital pharmacy, performed 10000 RNA-messenger injections in three months. Transcending the statutes through its public health mission, vaccination has thus emerged as a lever for city/hospital collaboration promising new partnerships and a pivotal role for the nursing home in its health area.
