In 1994, in order to investigate any future developments in AIDS up to 2010, a knowledge review was conducted using the scenario method. This prospective work made it possible to identify four strategic options: social control and financial logic, adopting a wait-and-see approach, organized health, or responsibility and freedom. This work, which was unusual in the health sector, emphasized the value of an organized health approach and upheld the principles of solidarity in the face of the stumbling blocks of an 'every man for himself' approach and disengagement from the community. Over the past fifteen years, major progress has been made in the fight against AIDS. In 2010, HIV infection remains epidemic so we need to plan and strengthen prevention, make therapeutic management and patient social support easier.
