The author examines how psychiatric institutions, because they are permanently confronted with the threat of disorganization, use defense mechanisms such as splitting that freeze the thinking process and exclude those who go against the shared denial. The author draws on caregivers-patients meetings in order to demonstrate how conflictuality is in fact necessary to psychic care. How can we think of the psychiatric institution as a border crossing with mixed occupation, that is to say, as a place that embodies the symptom, a place of confrontation and compromise between opposing forces?
