RésuméLes enfants sont moins souvent et moins gravement infectés que les adultes par le SARS-CoV-2. Une réponse immunitaire innée plus « entraînée », une immunité croisée avec les autres coronavirus, et une faible expression des récepteurs ACE2 peuvent l’expliquer. Peu de formes sévères ont été rapportées chez les enfants atteints de cancer. La poursuite du traitement anti-cancéreux est donc recommandée avec une adaptation chez certains patients (allogreffe, immunothérapie). En pédiatrie, à l’Institut Gustave-Roussy, 167 patients ont été testés par PCR (réaction en chaîne par polymérase). Dix (6 %) majoritairement a- ou pauci-symptomatiques étaient positifs. Aucun patient n’a eu de forme sévère, aucun cas nosocomial n’a été enregistré, et aucun membre du personnel n’a été infecté à l’hôpital. Une baisse de 45 % des nouveaux patients a été observée par rapport à la même période de mars à juin 2019. Les répercussions de l’épidémie ont été importantes en termes d’organisation et d’ajustements des pratiques, mais limitées pour les enfants et le personnel.

Children have a milder disease and a better prognosis than adults when infected by Coronavirus-2 (SARS-CoV-2). Pediatric oncologists have recommended continuation of anti-cancer therapy with adjustments for some patients (allogeneic stem cell transplant, immunotherapy). In the Gustave Roussy Paediatric Department, specific measures to protect children and staff, and prevent the spread of the virus, involve limited visits, phone consultations when possible, and teleworking. PCR tests became routine on March 23rd for patients requiring hospitalization. A total of 269 tests were carried out over four months in 167 patients. Ten patients (6%), mostly with mild symptoms, were positive. The number of infections among medical and paramedical staff was also very low. Very few children had delayed treatment. Relative to the period of March to June, a 45% drop in new patients was reported. However, the consequences of the SARS-CoV-2 epidemic on the department were significant in terms of organization and daily adjustment of practices.
