During medication reconciliation (MR), a structured medication review (SMR) is typically carried out by pharmacists, both internal and external to the pharmacy. At the Saint-Omer Regional Hospital, the time devoted to MR was insufficient, and daily deployment was impossible with the current staffing levels. However, other professionals such as hospital pharmacy technicians could perform this task. The French Society of Clinical Pharmacy recommends that subsidiarity be sought in the organization of this activity1.The objective of this work was to train a team of hospital pharmacy technicians in performing the SMR to develop and sustain daily MR activity. Prior to this, it was necessary to free up preparation time, reorganize the activity process, and provide internal training. A communication campaign was also implemented in order to create a favorable environment for the project. Over 9 months, various monitoring indicators were analyzed: the number of MRs performed, the number of unintended discrepancies (UDs) detected, the number of pharmaceutical interventions (PIs). A total of 120 MRs were performed by the hospital pharmacy technicians in collaboration with the pharmacist. Out of all the MRs, 68 % revealed at least one UD, resulting in 183 PIs, 71 % of which were accepted. The findings are consistent with the existing literature. The activity is now performed on a daily basis, with an increase in the number of MRs performed. Integrating the hospital pharmacy technicians enhances the value of their roles and diversifies their tasks, and helps develop their medical and pharmaceutical knowledge.
