Contrairement aux autres composantes traditionnelles du safety net, leurs objectifs initiaux étaient d’être des agents du changement social et non de simples fournisseurs de soins. Après une mise en perspective historique de leur émergence, liée aux mouvements sociaux des années 1960, cet article analyse leur évolution et le rôle que ces structures sont appelées à jouer après la mise en œuvre de la réforme de 2010. Une attention particulière sera portée aux free clinics, qui constituent une contribution méconnue au système de soins. |

Summary, etc. | In America’s market-oriented health care system, community health centers and free clinics provide care to the most vulnerable segments of the population. Born from a grassroots approach to solving social problems, their initial purpose was to be the agents of social change and establish health care as a human right. This paper provides an overview of the historical circumstances of their emergence in the 1960s and shows how they have evolved in response to changes in the political and economic environment. The study also raises the issue of their role after the implementation of the Affordable Care Act, with a special focus on free clinics, an overlooked aspect of the American social safety net. |
