| Summary, etc. | La mise en place d’un service de psychiatrie générale non-fumeur en 2004 au centre Henri-Rousselle (centre hospitalier Sainte-Anne) a reposé sur une demande institutionnelle et de la plupart des personnels et des familles ; il répondait à des exigences croissantes des instances administratives et légales. Les nuisances générées par le tabac, connues des usagers et des soignants, commençaient à invalider gravement l’approche globale de la santé dans cette institution de soin. L’absence d’expérience comparable en psychiatrie, la résistance des équipes et l’idéologie communément admise en psychiatrie sur le risque d’aborder ce problème nous incitaient à avoir des réponses tabacologiques techniques sur le sevrage, les effets pharmacologiques avec les psychotropes et, surtout, à ne pas se positionner dans le champ d’un hygiénisme de mode mais à inclure la question légitime du tabac dans le projet institutionnel des soins du service. |

| Summary, etc. | A non-smoking psychiatric health ward: the experience of sector 15 of the Sainte-Anne hospitalThe setting up of a non-smoking general psychiatric service in 2004 in the Henri-Rousselle centre (Sainte-Anne hospital) stemmed from a demand on the part of the institution itself and most of the staff and families; it also satisfied stricter requirements from administrative and legal bodies. The harmful effects of tobacco, well known to users and carers, were beginning to impact negatively on the overall health approach in this health institution. The absence of any comparable experience in psychiatry, along with the resistance on the part of teams and the commonly held view in psychiatry that it was risky to tackle this problem encouraged us to search for some technical information on tobacco withdrawal and its possible pharmacological effects when combined with psychotropes and, above all, not to fall into a kind of trendy hygienism but to incorporate the legitimate question of tobacco in the service’s institutional care project. |

| Summary, etc. | El lanzamiento de un servicio de psiquiatría no fumadores en 2004 en el centro Henri-Rousselle (centro hospitalario Sainte-Anne) fue debido a una demanda institucional y también por parte de la mayoría del personal y de las familias; respondía a exigencias cada vez mayores de las instancias administrativas y legales. La molestias generadas por el tabaco, conocidas por los usuarios y por el personal sanitario, empezaban a invalidar gravemente la dimensión global de la salud en esta institución sanitaria. La falta de experiencia comparable en psiquiatría, la resistencia de los equipos y la ideología comúnmente admitida en psiquiatría sobre el riesgo que supone abordar este problema nos incitaba a proponer respuestas tabacológicas técnicas sobre la desintoxicación del tabaco, los efectos farmacológicos con los psicótropos y sobre todo a no tomar posición dentro del campo de un higienismo de moda sino a incluir la cuestión legítima del tabaco en el proyecto institucional de tratamiento del servicio. |
